Rehab-Weight training? (Durham)

I am a middle-aged veteran man preparing for a tentative ACDF surgery and I am looking for an athletic trainer who may have experience in rehabbing patients with hand and forearm atrophy.
I am a healthcare contractor by trade and want to think outside of the box regarding rehab associated with the VA.
I have a membership to Planet Fitness, but for some reason, my old training methods are not providing lasting results.
I have an underlying issue that will eventually be addressed but I want to maintain the function that I have left at a high level.
I have lost about 60% of my hand strength bilaterally due to compressed nerves from spinal stenosis.
