Family/Internal Medicine Clinical Care Coordinator -FULL TIME, REMOTE (Full NC Licensed Clinical Mental Health Counselors Required) While this role is primarily remote (home-office based), we consider the Duke Primary Care Engagement Center our anchor site (where trainings, meetings may occur).

General Description of the Job Class: The Clinical Care Coordinator functions as an integral part of an interdisciplinary team, ensuring excellence in adult behavioral health care, in an effort to achieve optimal mental health outcomes through a seamless model of access and care.
Focus on improving access to behavioral health care for adult patients (18 years and older), providers, family and community.
Work Performed: · Provide behavioral health care coordination to parents and families in need of behavioral health services · Perform various appointment and access-related duties for DPC Behavioral Health to including scheduling appointments from a referral, following up on referrals faxed externally, and closing the loop on all reconcilable referrals placed.
The Clinical Care Coordinator remains current on behavioral health providers in the community, service locations, service offerings, and insurance payers, while providing the highest levels of service excellence to each patient · Responsible for eliciting the patient’s and family’s treatment preferences and needs and determining the most appropriate behavioral health services · Conduct patient contacts through telephone calls · Maintain liaison with other offices and departments to coordinate referrals, foster collaboration, leverage resources, and help to accomplish program objective · Contribute to program effectiveness by communicating trends and recommending modifications to current workflows · Actively participate in DPC continuous improvement processes and practice level improvement projects Knowledge Skills and Abilities · Licensed staff member with behavioral health training (e.

, Licensed Clinical Mental Health Counselor/Professional Counselor) · Will bill for behavioral health care coordination services incident to the primary care provider.
· Care management / care coordination experience required, CCM preferred · Provide accurate information to patients regarding various behavioral health services, providers, and locations available within the Duke University Health System and the surrounding community · Actively identify, listen, and understand the needs of each patient.
Ask thoughtful questions to determine the appropriate resources or actions to be taken · Professionally communicate both verbally and in writing with patients, providers, internal specialty departments, and clinics in the surrounding community · Organized and motivated by a fast-paced environment · Able to manage multiple tasks/projects simultaneously · Proficient in chart review and able to assess needs quickly · Strong with the use of computer software tools and data files · Comfortable with continuous change and is self-motivated · Able to complete documentation in a quick and efficient manner (will be in electronic health record and other software systems developed for care management and population-based program metrics) General Expectations · During the Orientation Evaluation (first 90 Days), it is expected that in-person training will occur with leadership and colleagues.
At minimum 2 full days in the office training, and additional days may be scheduled based on learning style and performance.
