The schedule for this position is Monday-Friday 8:00am-5:00pm Why you should join Transitions: **CME days** **We pay for license and credentialing** **6 weeks of paid time off (PTO) for 40 hour per week employees** **Mileage reimbursed** A Successful Advanced Practice Provider has the following: Caring and compassionate attitude with true concern for people Excellent interpersonal skills: ability to connect with patients, families, and internal/external partners Strong ability to problem-solve and communicate complex issues; ability to work collaboratively with all co-workers Exhibit ability to make responsible choice and decisions that are in patients’ best interest Responsibilities: Provide direct patient care on an ongoing basis as directed by and under the clinical supervision of the Chief Medical Officer, the Associate Medical Director and the referring physician, patient, family, and plan of care; utilizing pain and symptom management interventions that are based on established hospice and/or palliative care guidelines and standards.
Provide attending MD status for Hospice patients when assigned.
Provide Face to Face visits to ensure compliance with Hospice Medicare/Medicaid regulations Attend IDG meetings as appropriate Develop working knowledge of all federal, state and company regulations, policies, and procedures; ensure compliance Requirements: Nurse Practitioner with current NC license Preferred 3 years clinical experience Must have reliable transportation Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines What you can expect from us: Hiring Manager or Human Resources will contact you within 1-3 business days of your application submission Complete 1-2 interview(s) with the hiring manager via virtual video, or in-person Upon receiving the job offer, review job description, and complete new-hire tasks and paperwork (via Paycor) Complete New-Hire Orientation and clinical skills training Receive continuous training from our team as you continue to provide exceptional care to our patients Essential Functions: To perform this job successfully, an individual must be able to perform each essential duty.
