As one of the community’s industry leaders, we provide exceptional home health, hospice, and palliative care services throughout our 8-county territory.
Transitions LifeCare’s goal is to care for individuals and families with compassion, support, and expertise as they navigate the end of life or life changing illness.
The schedule for this position is 3-12 hour shifts per week 7:00am-7:30pm; weekends and holidays as scheduled.

A Successful Certified Nursing Assistant (CNA) has the following:

Responsibilities:
Provide high-quality and compassionate care to patients per their individualized care plan
Works collaboratively with the Hospice Home team in achieving patient care goals
Maintain comfort, privacy, and dignity of our patients
Promptly reporting pertinent information to a member of the leadership team.
Requirements:
Current CNA listing with NC Home Health Aide Registry
Must have reliable transportation
Minimum of 1 year experience
Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines
