What You'll Be Doing
Wake County Health and Human Services is looking for a Community Health Worker to join the HIV/STD Community Program team! Wake County's HIV/STD Community Program offers a comprehensive continuum of care including prevention education, HIV/STD counseling and testing, and treatment and supportive services for people with HIV/AIDS.
The Education and Outreach Team supports the HIV and Sexually Transmitted Diseases (HIV/STD) Community Program within Wake County Health and Human Services Department - Public Health Division.
The HIV/STD Community Program's health education and outreach team provides effective prevention services for a wide range of audiences.
Programs and classes are free and available in English and Spanish.
The Community Testing Counselor provides street and community outreach to reduce the risk of Human Immune-deficiency Virus (HIV) /Sexually Transmitted Diseases (STDs) by engaging high-risk populations into prevention programs and helping them to navigate essential services to address some of the social determinants of health that impact their behavior.
Ways this role will serve the community are: Guide individuals and community partners to adopt and support healthy behaviors through skill-based community education on HIV and STD prevention, transmission, and risk reduction strategies Create and conduct outreach programs to raise awareness on resources, distribute educational materials, and provide information on testing and treatment options - deliver educational messages around prevention, transmission and risk reduction Provide one-on-one contact, group contact, and contact with community organizations and nontraditional locations Support and maintain partnerships with stakeholders, residents, and local businesses Identify needs and refer individuals to Testing and Counseling services as well as provide education and resource referral to other Wake County Health and Human Services (WCHHS) services and resources - work closely with Outreach nurse with testing and treatment Ensure the delivery of culturally appropriate quality care and services; serve as the cultural liaison between programs and participants, community partners and human services programs Advocate with and on behalf of the community to ensure clients are treated equitably and respectfully and given access to the needed services and resources Additionally, this position will provide administrative duties such as- daily activities report, data entry, monthly reports, timesheets, and mileage.
Cultural sensitivity and understanding of diverse populations are important to address the unique needs of different communities.
Community Health Workers should have empathy and compassion to support individuals affected by HIV/STDs and collaborate with healthcare providers, community organizations, and local health departments to enhance the reach and impact of their efforts.

The Basics (Required Education and Experience) High school diploma or GED One year of experience in human services Equivalent education and experience are accepted How Will We Know You're 'The One'? Basic knowledge of HIV/STD, substance abuse, community building and community resources Possess excellent interpersonal skills, ability to build and maintain positive, productive relationships with peers, colleagues, and supervisors/management Confidence in working with at-risk and diverse populations and enjoy working in a team environment Demonstrate a high degree of adaptability, comfortable with work changes and new directions Ability to convey HIV/STD and health information clearly and concisely to individuals and groups Ensure they understand the information taking into consideration client's health literacy Provide excellent customer, client services skills including the ability to listen and respond appropriately to others and the ability to maintain confidentiality and conduct with tact and courtesy to others Basic computer skills and experience developing educational materials and general understanding of technology to provide education About This Position Location: Human Services Center Sunnybrook Raleigh, NC 27610 Employment Type: Regular Work Schedule: M-F 8:30 am - 5:15 pm Occasionally nights and weekends based on community needs Hiring Range: $19.
